Directors are responsible for sending all applications for each school in one envelope accompanied by one of the following: 1) ONE SCHOOL or TEACHER WRITTEN check or money order for the total amount due or, 2) a copy of the Purchase Order.  Do NOT send cash or parent/student written checks.  Certified mail is recommended. 
Purchase orders for audition entries and/or festival registration participants must be paid by APRIL 1 of the festival year in order for students from that school to participate in subsequent festivals.  Please list the students by voice section on separate pages and return with this page on top.


Applications must be postmarked on or before Wednesday, October 1, 2008 and must include the audition fee of $10.00 per student entry. Late applications or changes postmarked on or before Wednesday, October 8, 2008 will be accepted only if they include a double audition fee of $20.00 per student.  Entries postmarked after October 8th will be charged an additional $100 penalty fee.  NO EXCEPTIONS.  Only students who are currently enrolled in grades 10, 11, or 12 may audition.  Auditions will be held on Tuesday, November 18, 2008, at First Baptist Church, Mustang  (928 N. Mustang Rd.).

I recommend the students listed as persons/performers suitable for membership in the All-OCDA High School Jazz Chorus. I understand that my students may audition only if I am present at the audition site and that I am a current member of ACDA/OCDA. I also certify that a student will not audition if he/she is academically ineligible according to OSSAA rules. I understand that I am required to work at the audition site for the entire day and will make arrangements for my students to be transported and supervised to and from the audition site.

I certify that these students are eligible to audition for the All-OCDA High School Jazz Chorus. This eligibility meets the requirements set by the OSSAA. I understand that in order for these students to audition, my vocal music teacher is REQUIRED to work at the audition site for the entire day and that he/she will make arrangements for the students to be transported and supervised to and from the audition site.
