Please return by Friday, February 6, 2009, with registration fees.  Print this form for each quartet submitted, marking which quartet, if any, is your optional third quartet.  Each school may send up to two balanced quartets, with a third quartet as optional based on need.  Singers are selected at the discretion of the director.  Schools participating will be able to substitute students if needed with prior notice.  Cost is $125.00 per quartet.  Make checks payable to OCDA.  .
